What is Transition Care?
Transition Care helps you to settle back into home after a hospital stay.
We can provide short-term support and care while you recover and regain your independence. These supports include:
- Allied health therapies
- Nursing support
- Personal care
In collaboration with your hospital team, our Care Partners will design a personalised program based around your needs for up to 12 weeks.

How do I access Transition Care?
To be eligible for Transition Care, you need to be aged 65 or over (or 50 and over for First Nations people).
Here’s how to access Transition Care:
- You or your hospitall team can contact My Aged Care and arrange an Aged Care assessment (ACAT).
- The ACAT assessment will determine your eligibility.
- Get in touch with UnitingSA by calling (08) 8448 6280 or emailing homecare@unitingsa.com.au. We can assist you to leave the hospital and begin receiving supports.
