How does the Tailored Learning Program work?

Our Tailored Learning Program can help young people get back into learning, training and exploring employment pathways.

Our program is designed for young people aged 12 to 21 who are currently disengaged from mainstream education.

Through strong partnerships with schools, families and local services, we deliver flexible support that focuses on both learning and wellbeing.

Our dedicated team includes a Coordinator, Senior Case Manager, Case Managers and an Aboriginal Case Manager.

Together, we work collaboratively to provide personalised support that empowers each young person to thrive.

Please note: Tailored Learning referrals can only be completed by the school of enrolment. If seeking additional case management support for a young person, Urban Youth may be a suitable option.

How can we help?

Through Tailored Learning, we can support participants with the following services:

Case Management
1:1 assertive outreach case management for young people who are disengaged from education

Fee-For-Service Engagement Program
Engagement and capacity building programs (small group programs aimed to increase confidence, peer connection and serve as a stepping stone to further learning)

Supervision
External supervision (1:1 and/or group supervisions offered to school employed youth workers. Sessions are intended to create a safe, reflective space to discuss client issues and enhance practice).

FAQs

A: Yes. All young people in the Tailored Learning Program remain enrolled at their home school.

A: Referrals must come through the young person’s school of enrolment. If you believe someone would benefit, speak to their school about making a referral.

A: No. The program is fully funded by the Department for Education and the school of enrolment, so there is no cost to families.

A: We offer flexible, individualised support to help young people overcome barriers to learning. This includes:

  • One-on-one case management
  • Educational goal setting
  • Career and pathway planning
  • Life skills development
  • Referrals to external services

A: Yes. All students in Tailored Learning are considered full‑time students, regardless of their personalised timetable.

A: Young people aged 12‑21 who are disengaged, or at risk of disengaging, from education. Eligibility depends on individual needs and is determined in partnership with the school.

A: We value the involvement of parents and carers, but tailor our engagement based on what works best for the young person. Consent and confidentiality are discussed during intake.

A: Yes. We offer assertive outreach, meaning case managers can meet young people at home, in the community, or provide transport to case management appointments. Over time, we aim to support increased confidence and independence, however our model allows us to meet young people where they are at.

A: Our team includes experienced case managers with backgrounds in youth work and social work, an Aboriginal Case Manager, a Senior Case Manager, and a Youth Services Coordinator.

A: We support young people to plan for what’s next — returning to school, starting work, enrolling in training, or linking with other services — to ensure a smooth and supported transition.

A: We use a client‑centred, strength‑based, trauma‑informed and culturally responsive approach. We also incorporate Restorative Practice, focusing on strong, respectful relationships.

A: No. A formal diagnosis is not required. We support young people with a range of learning, wellbeing, or personal challenges.

A: We maintain regular communication and reporting with schools. This includes weekly attendance updates, case reviews, funding requests, and co‑developed education plans with the school, family, and young person.

A: Yes. We welcome collaboration with schools, RTOs, and service providers to improve outcomes for young people. Please feel free to get in touch with us to discuss further.

A: Yes. All staff undertake regular cultural responsiveness training. We have a dedicated Aboriginal Case Manager and work closely with Aboriginal services to provide culturally safe and meaningful support.

A: At our southern site, we offer programs like Supported Learning, Wood Works, Pre‑Employment, Life Skills, and Wellbeing. We also deliver short courses such as First Aid, White Card, and Barista training.

We provide group and 1:1 supervision to schools with in‑house youth workers, even if young people aren’t linked to our case management.

A: Yes. Many schools we support deliver Tailored Learning internally and still access our professional supervision services for staff.
